General Guidelines:
- Use this form to request or delete access to the Legacy SciQuest system.
- The following roles can submit SciQuest requests:
- Business Partner (Operations Manager)
- Strategic Business Partner (Lead Administrator)
- Requests can be submitted for oneself or for a single user.
Actions to Locate the Legacy SciQuest Request
- Navigate to the Procure-to-Pay Functional Support service page. NOTE: If you do not have access to submit this request an Item Not Found screen displays:

- Click on the blue Request Service Button.
- Under the How Do I Get It? section click the Legacy SciQuest Access Request link.

Actions to Complete the Request Form:
- Complete the Request Details section:
- Requested By – Requester's name will automatically populate.
- Ordering For – Select one of the following from the drop-down.
- Myself – Select when submitting a request for yourself.
- Single User – Select when submitting a request for another person. Additional fields display:
- Requested For – Input the person’s name the request is for, or select their name from the drop-down list.
- NetID – This field will automatically populate based on the selection made in the Requested For field.
- Email - This field will automatically populate based on the selection made in the Requested For field.
- Phone - This field may automatically populate based on the selection made in the Requested For field. If the field does not populate, input the Single User telephone number.
- Proceed to the Request Type options of Add, or Delete.
Actions to Complete the Add Request Type
- For the SciQuest Request Type click the Add radio button.
- Complete the additional fields listed in the Request Details section:
- Why do you need access to SciQuest (Legacy) data? -Add a comment about why you are requesting access to Legacy SciQuest Data.
- Add attachments as necessary by using the paperclip icon.
- Click Submit and the request is routed to the proper team to fulfill the request. The submitted request will be displayed in My Items at the top of the IT Service Portal in the Active Tickets section. The Requester will also receive an email notification and can track the status by clicking the request number link.
Actions to Complete the Delete Request Type
- For the SciQuest Request Type click the Delete radio button.
- Review the Warning section.
- Add attachments as necessary by using the paperclip icon.
- Complete the Special Instructions Section.
- Comments (optional) - Input any additional information that could offer further assistance when fulfilling the request.
- Click Submit and the request is routed to the proper team to fulfill the request. The submitted request will be displayed in My Items at the top of the IT Service Portal in the Active Tickets section. The Requester will also receive an email notification and can track the status by clicking the request number link.
